Use the pre-unwind-handler of a catch clause instead of nesting
with-throw-handler inside a catch clause.

* src/web/server/fiberized.scm (with-ignored-disconnects): Remove it,
(client-loop): replace "with-ignored-disconnects" with a catch clause.
